WELL CHILD CHECK UP TEACHING SHEETS

I give these sheets out at the corresponding check up. All pediatricians know that parents tend to ask similar questions at well child check ups. Those questions are different depending on the age of the child. We all know how easy it is to forget some of the details of those answers simply due to how much information we discuss and the stress of having your child examined and given immunizations. Its kind of like going to the store and forgetting half of what you went there for. I took the most common questions that parents ask at each age, wrote down as complete an answer as I could think of and created my teaching or well child check up sheets. I added what I felt was important information that parents may not ask, such as car seat safety, when to start vitamins, when the next check up is due and what vaccines may be given at that visit. I stopped making routine sheets after the 4 year check up as the questions that people ask become too diverse.
